What is Support Coordination?
When you’ve got an NDIS plan, choosing service providers is a great power, but it can come with its challenges – and that’s where Support Coordination comes in!

Think of a Support Coordinator as your personal NDIS guide. They’re here to help you navigate your plan, connect you with awesome people and services in your community and ensure you’re making the most of all the fantastic support options available to you.

How do I access Support Coordination?
Is this service in my NDIS plan?

In your NDIS plan, support coordination is funded by the Capacity Building Supports budget, under ‘Support Coordination’ or ‘psychosocial recovery coach’.

How can a Support Coordinator help me?
Your Aucare Support Coordinator can help you:

  • Find providers for the supports you’re funded for
  • Achieve your goals
  • Access healthcare and education services
  • Participate more in your community
  • Look for a job
  • Work through your accommodation needs
  • Develop the confidence to coordinate your own supports
What support coordination services does Aucare provide?
Through the NDIS, there are 3 levels of support coordination that can be included in your NDIS plan.

At Aucare, we offer Level 1, Level 2 and Level 3.

Level 1: Support Connection
Support to help a participant understand their plan, connect with NDIS providers and community, mainstream and other supports to get the most out of their NDIS plan.
Level 2: Coordination of supports
With Level 2, we can help increase your knowledge and confidence in navigating the NDIS and your plan. We can:
  • Connect you to NDIS and other government services.
  • Help you negotiate supports within your plan budgets or gather evidence to request a new NDIS plan.
  • Monitor your plan spend together and upskill you on the different types of plan management.
Level 3: Specialist Support Coordination (SSC)
It’s like Support Coordination but with an extra touch! Our SSC team members, with backgrounds like social work, are ready to:
  • Tackle challenging service situations alongside you.
  • Develop plans and contingencies for more support options, especially if your needs change regularly.
In every aspect of Support Coordination, we’re here to guide you to set priorities, manage services, connect with opportunities, and truly understand your plan!
Aucare offer psychosocial recovery coach
Psychosocial recovery coaches support individuals with psychosocial disability to recognise, reflect and take responsibility of their own recovery.

We work collaboratively with individuals, their families, carers and other services to design and implement a recovery plan and help with the coordination of other NDIS supports.

Individuals can choose to work with our recovery coach who has learned or lived experience of psychosocial disability.

What does a recovery coach do?
Recovery coaches work with individuals with psychosocial disability to:
  • Develop a recovery-enabling relationship
  • Increase awareness of choice and control
  • Identify individual goals and develop strategies to achieve them
  • Provide ongoing support and engagement (even in times of increased needs due to variation in individual mental illness)
  • Build individual capacity, increase recovery skills, resilience and decision making.
